Benefit summary gives an overview of the benefits provided by health insurance plans and does not include all requirements or qualifications of each benefit. It shows current rates for all benefit programs including health, dental and life insurance. It is only a document given for informational purposes and does not constitute an agreement.
Additional information regarding other requirements and specific benefit maximums will be provided to enrolled members in a plan document handbook. In case of conflict between information contained in the benefit summary and the official plan documents, the official plan documents will govern.
