Chronic Care Improvement Program Law and Legal Definition
Chronic care improvement program is a program designed by the chronic care improvement organization to improve clinical quality and beneficiary satisfaction and to achieve spending targets with respect to expenditures for targeted beneficiaries with one or more threshold conditions. Generally, chronic care improvement programs are offered under an agreement. [42 USCS § 1395b-8 (a) (2) (A)].
