Level three services refer to the outpatient and specialist services. Services that fall under level three services are outpatient hospital services such as lab, radiology, outpatient surgery, physical, speech, and occupational therapy, emergency room service provided to patients who are not admitted, ambulatory surgery center, surgery service offered in physician’s office, therapy services, and outpatient mental health services
