Resident Assessment Protocol (RAP) Law and Legal Definition
Resident assessment protocol (RAP) refers to the documents that form part of the resident assessment instrument. These documents will contain information about many health care topics, including pressure, ulcers, or asthma. It was adopted as a measure to help in the planning of care of frail elderly residing in nursing homes. It helps in determining which persons are at the risk of specific functional disabilities. It is a means for arriving at resident assessment and problem identification. RAP will provide a statement about the health problem, then the minimum data set (MDS) will trigger based on that information guidelines about treatment.
