Threshold Condition (Health Care) Law and Legal Definition
Threshold condition in the context of health care means a chronic condition such as congestive heart failure, diabetes, chronic obstructive pulmonary disease (COPD), or other diseases or conditions. The Secretary of Health and Human Services will chose the appropriate threshold condition for the establishment of chronic care improvement program.
